We are a person-centred and specialist Homecare provider within Hampshire and the Isle of Wight.
We provide a high quality, flexible and professional service to adults and young people with a variety of complex needs.
The Outreach service is outcome focused and driven to support individuals to work towards and achieve independence through focused core teams of staff who are caring and well trained. The key is to be person led, and that is what we do, from the assessment and core team building process, through to setting targets, our approach to the support and measuring peoples progress.
We support individuals to access meaningful community activities, employment opportunities and gain experience in the voluntary sector. As part of the support that we provide, individuals choosing to utilise our services can access Willow Village and will have a range of enriching and social sessions available to them. This can include gardening and growing vegetables, specific ASDAN programmes and other daily sessions run within this inclusive site.
We support individuals in their own home with living independently, personal care washing/dressing/medication and eating/drinking] budgeting, cleaning, cooking and attending health appointments. The service will work with the individual and relevant others to ensure a clear person-centred support plan to ensure that the person is being empowered to achieve optimum independence for them. Support may be within a supported living environment, own home, respite, a Residential provision requiring additional support or a holiday location.
The service can offer family respite within the home environment including night support where this is required.
The Outreach service have the ability to support with transition planning for individuals leaving more secure settings such as hospitals and specialist residential locations.
The Outreach Service is dedicated to providing positive, pro-active, calm and caring support which is specifically designed to empower individuals to live life happily and achieve their own personal goals.
